In order to take advantage of a the benefits of an Health Savings Account, you need to be enrolled in an HSA-eligible health insurance also referred to as a High Deductible Health Plan (HDHP). After you are approved for insurance, you can open an HSA account with SFCU. Remember, a Stanford FCU HSA is available to anyone, not just Stanford employees.
If you are already a member, just print and complete the HSA Custodial Application, the Stanford Federal Credit Union HSA Agreement and the HSA Contribution Form. Please make sure to designate at least one beneficiary and sign all forms.
To transfer your existing Health Savings Account from another institution to Stanford Federal Credit please complete the following forms, HSA Direct Transfer Form , HSA Rollover and Transfer Form, HSA Custodial Application and the HSA Agreement . Please submit all of these forms with a copy of your current Health Savings Account statement.
If you're not a member, you will also need to complete our online membership application.
Once you have competed the HSA Custodial Application, the Stanford Federal Credit Union HSA agreement and the HSA Contribution Form, return them to SFCU, keeping a copy of these forms for your records.
